Responsibilities include: care/case management services, in-home assessments, grant writing, advocacy/activism, strategic planning, community outreach, committee and volunteer development. Ideal candidate will possess a bachelor’s (required) or master’s (desired) degree in health or human services and 2-3 years of demonstrated experience in a related field. Strong organizational and computer skills are a must. Some travel, evenings and weekends required.
